What Is Frozen Shoulder?
Frozen shoulder happens when the connective tissue around the shoulder joint, the capsule, becomes inflamed, thickened, and stiff. Over time, this limits movement and causes significant pain.
Frozen shoulder typically goes through three stages:
- Freezing Stage (Painful Stage): Pain starts gradually and worsens over time. As pain increases, shoulder mobility begins to decrease.
- Frozen Stage (Stiff Stage): Pain may lessen slightly, but stiffness becomes the dominant issue. Movements like reaching overhead or behind the back become extremely difficult.
- Thawing Stage (Recovery Stage): Shoulder range of motion slowly returns, and pain continues to subside.
This whole process can take 12 to 18 months on average, with some cases lasting over two years.
Who Gets Frozen Shoulder?
Frozen shoulder is most common in adults between the ages of 40 and 60, and it tends to affect women more than men. There are also a few known risk factors:
- Diabetes (especially Type 1)
- Thyroid disorders (both hypo- and hyperthyroidism)
- Post-surgical immobility (like being in a sling after shoulder surgery)
- Previous shoulder injury

Case Study: A Long Road to Recovery
One of our current patients, a 55-year-old woman with Type 1 diabetes, is experiencing frozen shoulder for the second time. Her first case affected the right shoulder and took nearly 18 months to resolve with physical therapy and cortisone injections.
Now dealing with it in her left shoulder, she initially tried to manage symptoms on her own, but after a flare-up during vacation, she experienced a major setback. Since then, we’ve been working closely with her using a multi-modal treatment approach including:
