Shockwave Therapy vs. Class 4 Laser Therapy: What’s the Difference (and Which Is Right for You)?

Shockwave Therapy vs. Class 4 Laser Therapy: What’s the Difference (and Which Is Right for You)?

If you’re dealing with pain, whether it’s a stubborn case of Achilles tendonitis or a neck strain that flared up overnight, you’ve probably heard about advanced therapies like shockwave and laser.

At Team Elite Chiropractic, we use both Shockwave Therapy and Class 4 Laser Therapy to help athletes and active individuals heal faster, reduce pain, and get back to doing what they love.

But what’s the difference? And how do you know which one is right for you? Let’s break it down.

What Is Shockwave Therapy?

At Team Elite, we use the Storz Medical D-Actor radial shockwave system.

Shockwave therapy works by sending high-intensity sound waves into the body. These aren’t electrical shocks, they’re pressure waves that create controlled microtrauma in the tissue.

That controlled stimulus triggers a process called mechanotransduction, which activates your body’s natural healing response.

What Shockwave Therapy Does:

  • Stimulates new blood vessel formation (angiogenesis)
  • Encourages stem cell activity
  • Breaks up chronic adhesions and calcifications
  • Promotes tissue regeneration
  • Reduces pain

What It Feels Like

Shockwave feels like a deep, intense tapping sensation. In certain areas, especially where your pain is located, it can feel sharp or achy, similar to a deep tissue massage that “hurts so good.”

It’s not typically described as comfortable, but it often feels like it’s hitting the exact spot that needs treatment.

We adjust the intensity for each patient and aim for about a 4-6/10 discomfort level. It should be tolerable and therapeutic, not overwhelming.

Best For: Chronic, Stubborn Conditions

Shockwave really shines with long-standing, degenerative issues like:

  • Tennis elbow
  • Achilles tendinopathy
  • Plantar fasciitis
  • Chronic shoulder pain
  • Patellar tendonitis
  • Long-term neck or low back pain

Think of shockwave as the tool we use to “wake up” tissue that hasn’t responded to rest, stretching, or traditional treatments.

How Many Sessions?

  • Acute tendinopathies: ~6 sessions
  • Long-term degenerative issues: 6-12 sessions
  • Maintenance (for ongoing support): every 2-4 weeks as needed

Who Should Avoid Shockwave?

Shockwave is safe for most patients, but we avoid it in cases of:

  • Active cancer in the treatment area
  • Pregnancy (over abdomen/lower back)
  • Acute infections
  • Within 6 weeks of a cortisone injection
  • Caution with blood thinners or bleeding disorders

It can safely be used around metal implants like joint replacements.

What Is Class 4 Laser Therapy?

We use the Summus Platinum P4 Class 4 Laser, a high-powered therapeutic laser designed for deep tissue healing.

Class 4 lasers are stronger than Class 3 “cold lasers.” They penetrate deeper and produce a warming effect.

How Laser Therapy Works

Laser therapy works through a process called photobiomodulation.

The laser light is absorbed by the mitochondria in your cells, increasing production of ATP (your body’s cellular energy source). More ATP means:

  • Faster tissue repair
  • Reduced inflammation
  • Improved circulation
  • Decreased pain

It also gently heats the tissue, which increases blood flow and promotes healing.

What It Feels Like

Laser therapy typically feels warm and soothing. Most patients find it comfortable and relaxing.

We keep the laser moving continuously to avoid overheating the skin. Protective goggles are worn during treatment.

Best For: Acute & Nerve-Related Conditions

Laser therapy is especially effective for:

  • Sprained ankles
  • Muscle strains
  • Ligament injuries
  • Acute neck pain
  • Hamstring pulls
  • Sciatica
  • Carpal tunnel syndrome
  • Numbness and tingling

If you “slept wrong” and your neck is locked up, or you just rolled your ankle yesterday, laser is often our first choice.

How Many Sessions?

  • Acute injuries: ~6 sessions
  • Chronic conditions: up to 12 sessions

Who Should Avoid Laser?

We avoid laser therapy:

  • Directly in the eyes
  • Over active cancer
  • Over the abdomen during pregnancy

It’s safe for most other patients.

Shockwave vs. Laser: Side-by-Side Comparison

One way to think about it:

  • “Wet” injuries (new, inflamed, swollen) → Laser
  • “Dry” injuries (old, degenerative, stubborn) → Shockwave

Both treatments penetrate deeply. The difference isn’t depth, it’s the type and stage of tissue damage.

Can You Combine Shockwave and Laser?

Yes, and this is where the magic happens. They are complementary, not competing treatments.

Shockwave can create the stimulus that restarts healing in chronic tissue. Laser then enhances the environment for that tissue to repair more efficiently.

Using both often produces faster and more complete results than either alone.

If time, recovery speed, and performance matter (especially for athletes), combining them can significantly shorten your total recovery timeline.

Is It Worth the Add-On Cost?

Shockwave and laser are considered advanced add-on therapies. However, many patients find that:

  • Fewer total visits are needed
  • Recovery happens faster
  • They return to sport sooner
  • They avoid injections or more invasive options

In many cases, investing upfront reduces long-term treatment costs. For competitive athletes, or anyone training for a race, season, or event, speed matters.

Which One Is Right for You?

If you’re dealing with:

  • A long-standing tendon issue that won’t go away → Shockwave
  • A fresh injury or nerve pain → Laser
  • A chronic condition that needs acceleration → Both

The right choice depends on your condition, your timeline, and your goals.

At Team Elite, we assess your injury thoroughly and create a customized plan designed to get you back to peak performance as efficiently as possible.

If you’re tired of waiting for pain to resolve on its own, let’s take a proactive approach.
